When dropped into underworld lava while the Guide town NPC is alive, the doll consumes the Guide and instantly summons the Wall of Flesh. This sacrifice serves as the necessary rite transitioning the world into Hardmode, requiring players to ensure the Guide is alive in a valid house before initiating the battle. Players must ensure the Guide is alive in a valid house before making the sacrifice in the fiery underworld depths, as absent NPCs prevent the boss from awakening.

NPC vulnerability mechanics and cosmetic farming

Equipping the doll in an accessory slot allows players to attack the Guide directly with swords, arrows, and spells. Defeating the Guide with direct player attacks drops the Green Cap if his name is Andrew. If a Voodoo Demon is slain over lava and drops the doll into magma, the Wall of Flesh awakens immediately, creating sudden hazards during cavern farming runs across the molten underworld. This vulnerability enables targeted farming of Guide drops and allows manual relocation of his housing assignment without waiting for hostile monster invasions.
